But if a landscaper just builds the slabs up to the door track and leaves it, the rain has nowhere to go. It breaches the damp proof course. The water just pools up, runs straight under the doors, and ruins the kitchen floor.

You can't just pave up to the brickwork at that height without thinking about the water.
We fit proper threshold drains right across the back of the doors. It sits flush with the paving, but it gives the heavy rain somewhere to drop into before it ever touches the house.
Engineering the Falls

We engineer the falls so the patio pitches away from the building. You still get that flat walk-out from the inside, but the surface water is actively running away from the brickwork.
